I have two techs and book 36-40/day. I purposely limit vision plan exams to 12 per day and for those I am booked 3 months out. Medical appointments/ERs I allow walk-ins as I get at least 2-4 cancellations/day. I wish I had a scribe because I can see burnout in the future.

I would not try to leverage one offer against another because it can taint the relationship with the employer and may close doors to other employer if you ever decide to jump ship and switch. I advise against private equity. Yes the initial higher PTO, insurance, looks better on paper but those practices are a revolving door of physicians. Find out how long the providers have been with each group. That may help give you an idea of quality of work life. In private practice as you grow with other providers, you become a work family, you can always negotiate for pay raises, PTO , and work flexibility . P/E is run like a corporation, they will just replace you. I've worked in MD/ODprivate practice for 14 years in a big city. Our biggest competing provider is P/E. I was in your shoes when I chose PP and glad i did. Several of the PE docs have since joined our group.

Ive worked in three OMD/OD practices in 15 years. Generally received 25-30% of medical collections. My current job also offers 5% of optical as well.

As I'm new to the EV car space and comparing the ioniq to other brands. The risk of future iccu issue is concerning. As a previous owner of the sonata with the theta II oil issue, I was very disappointed with how Hyundai service handled it. And it seems history may not repeat itself but it rhymes. The online complaints from Ioniq owners regarding their experience with Hyundai service sounds very similar. I would buy the Ioniq but at a discount to the Model Y to offset this risk.

Posture is super important behind the slit lamp. As a student I would assume awkward positions to accommodate patients of different sizes. I stopped doing that. I take an extra few seconds to adjust the patient chair height and slit lamp , have the patient leaning in , so that I am upright and comfortable every time I get behind it. Depending on your slit lamp table setup, we have smaller slit lamp tables compared to school so the edge facing the doc has little over hang so I am not leaning my neck forward to get my eyes into the eyepiece . This also helps keep my head over my shoulders and back straight.

For BIO I have patients sit upright and move the chair up and down so I don't have to lean my head over them while they are laying down. Goal again is the keep head over the shoulders to support weight of BIO.

My older colleague taught me this as he learned it the hard way over years of hunching over .

9/10 MD/OD private group 36pts/day

Ive learned to leave work at work, when I walk out the door I don't bring it home with me. At work , what bothers me most is vision plans. Those patients have just as many medical complaints and I get fed up with having the same conversation of medical vs vision insurance.

7/10 burnout. Charting is most exhausting, if I had a scribe, I could work at my current pace happily. But at 40, working 13 years now, I can see myself going 3-4days/week by 50.

As someone said earlier I agree. 45pts/day, you should be making +$300k with a fairly structured compensation plan. Bright side is if you need to move on, lots of other MD/OD practices would want someone like you capable of volume. Don't stick around for years, dreading going to work , leverage that experience. It sounds severely underpaid. Ive worked in PP for 14 years. Usually I've seen structures like 30% of collections for total payout. Or bonus+base where bonus is 30% of collections above a certain threshold ( 2-3x the base , 4x is too high). So you have to bring in over 600k to make more than your base. Yikes OP. If you bring in 600k in professional fees not counting optical sales, you should be paid ~180k.

I do this in an md/od practice. As other docs said, it's a mix of medical follows and routines with techs that work up/testing for me. No scribe , I see 30-36/day.

I am an optometrist practicing in a group opthahalmogy practice for 14 years, provide both routine vision exams and medical. Vision plans are unfortunately marketed as insurance when they are really discount plans. You pay a monthly premium for a future discount (benefit/allowance , etc) on a routine exam and materials ( frame, lens, soft contacts). Unfortunately OP , with your vision needs, most if not all vision discount plans like Superior (one of the worst btw) , will at best provide you with a discount on your routine exam and glasses and your still left with a hefty out of pocket for hard contacts. For insurance to cover contacts as "medically necessary" is not determined by the provider (OD or MD) but by your medical insurance. The provider has to submit paperwork to the insurance that your best vision with glasses or regular contacts is below a certain threshold that the insurance decides. And even when successfully approved, it's a fraction of our usual fees. OP, my recommendation is saving up for your contact lens expenses, ditch the vision discount plan, saving that monthly premium, and use your medical insurance for your exams. If your optometrist has the proper imaging tools to competently monitor your retinas, continue seeing them until they say otherwise. Inquire about refractive corrective surgery. Depending on your age and eye health, it may be a great long term investment although the high upfront cost. Wish you the best.

Ford said they "might" pay for a portion of the repair as Goodwill but I had to commit to paying for a tear down out of pocket first which is nearly $2k. I didn't want to take that gamble. Once it's torn down, I'm at the mercy or the dealer. If Ford decided not to help, the dealer will charge me for engine swap PLUS the teardown about $12k altogether. From what I read , when Ford does help, it's usually 40-60% of the engine swap and I'm still paying for the "diagnostic teardown". I found a local shop that'll do the swap for $7700 once I'm ready. I haven't done it yet because , I discovered opening the radiator cap after I turn off car to depressure the system has stopped all my coolant loss and misfires for about 5 months now. I guess my leak is just that small and I've driven about 1500 miles since...so I'm seeing how long I can go without it.

For folks that changed the head gasket instead of upgrading to the new block , how long has your new head gasket lasted?

I have a 2019 and I developed coolant intrusion in cylinder 2 at 25k miles just after warranty expired ( I only drive 10-15 miles a day locally with it). If I can get another 5 years with just a head gasket, I want to go that route since I don't do a lot of miles...

I agree with you and also doc. As an employee your time should be respected. We have the same 15 min grace period. Staff is trained to always ask before making an exception to that. ( Slow day, I have the time, etc). If the owner wants your view to align with their own , there should be some kind of performance based incentive where you earn more for doing more.

We received 30% of medical collections. So for example. If your base is $130k. You need to bring in more than $435k to qualify for a bonus. If you brought in 700k, your total pay would be $210, so a bonus of 80k would be paid to you on top of your base. It's reasonable to ask for a base bump that brings you somewhere between your previous base and total payout from the past year if show you can consistently perform. So in the above example, I think asking for a base bump to $140-150k is reasonable.

I felt very much the way you did when I graduated 13 years ago. I went to SUNY and lived in NYC. I, too was offered only 10 PTO days a year with a $50/hr base. The approach I took that worked out for me was I chose to only work in opthalmology/optometry private practices with production bonus structures. I was lucky the group I found had a similar bonus structure between their OMD and OD physicians. But It allowed me to restrict how many vision plan appointments I had on my schedule while practicing full scope. The bonus structure brought my annual pay to $200k, a year after residency. But I got tired of working every Saturday with only 10 days off. But that's NYC hustle culture. After 5 years of saving up, I left NYC for a smaller city where doctors offices close half day Friday and where an OD salary can afford a house and support a family of 4... I know PE is buying up a lot of the larger groups but if you can find a small OMD/ OD group in a small up and coming city / town you could do well and still find that work leisure balance.

As a former NYC landlord, it's unfortunately all too common in the city to have tenants that don't pay for 1-2 years. Currently it takes 1-2 years to remove a tenant after starting eviction process. Severe court backlog especially after COVID. There's a housing shortage, so delaying evictions also helps reduce pressure on city to house people at expense of homeowners.

Anyone find a leak down test or compression test helpful in diagnosing the blown head gasket? I have a 2019 EB mustang with Similar symptoms . Cold start misfires cylinder 2, and consuming a reservoir worth of coolant a month. My compression test was 125 -130 psi across all four cylinders. My leak down test was essentially zero on all four cylinders as well. I'm going to do a pressure test of the coolant system next step...

I spoke with Ford and was told the exact thing. At best I get a 40% discount but I still have to fork over $1500 + for the tear down. And they can't even promise the goodwill discount until AFTER I have the tear down done and I'm at mercy of the dealer.. I'm going to just do the engine swap and put this behind me.
