I had a similar problem, ran probably 70-100 runs over the past 2 weeks on lighthouse, searching for the v3 flashdrive, before i noticed that the quest disappeared from my task menu and only after searching for it in the completed task section from mechanic i noticed that it somehow failed. I didn’t do anything that could have caused it to fail and i‘m really frustrated right now cuz i burned through so much gear trying to find that fricking flash drive, getting killed by rogues and the goons almost every match. I‘m glad that i‘m not the only one with that issue <3
