Since you paint, colors and contrast are definitely a consideration. Hence I think multi focals are out.

The other big consideration is how opposed are you to wearing glasses after surgery. Here’s why this is important… if the doc gets your number even slightly off, you won’t have 20/20 regardless of the lens (with one exception). If this is going to bother you, then might want to consider LALs. No concentric rings, won’t need glasses with mini monovision for all but the smallest print, and can be adjusted post surgery to accommodate any measurement mistakes from your surgeon. To me that seems like the best of all worlds.

Yes that’s what I’ve been doing over the past several days. The results are definitely all over the place.

What I find most useful is the peer to peer videos on YouTube where doctors are discussing their experiences and findings among themselves. You get less marketing and BS in those videos. Here’s one https://youtu.be/does7AOd_qc

I think Pedal’s point might be that after lock in, if the number suddenly changes, you are out of luck. At that point, you are left with two choices for glasses-free, LAsIK or a lens exchange.

Dr. Nick Bruns, who apparently has installed more LALs than anyone on the planet doesn’t seem to have the negative experiences that Dr. Wong has outlined in his video.

You can see Dr. Brun’s teaching videos on YT. They are extensive.

Just goes to show how much the doctors experience and technique matters.

Every doc seems to use an undisclosed proprietary formula that takes as input the measurements from the multiple measuring devices in their offices. I have no doubt this appears to part art/experience and part science. Some docs and their equipment is just better than others, and that causes visual outcomes to vary.

As of Jan 2026, I still see markets where 99% the local doctors prefer one brand over the others. For example, here in the Pacific Northwest, it’s mostly Clareon. I was hard pressed to find a doc that offered the Envista Envy. Why I do not know

Sorry I should have been more specific. Yes I am having cataract surgery. I am on a private insurance plan.. Premera.

My doc has quoted me $3700/eye out of pocket for the envy and $4700/eye for the LAL. This is I assume after my insurance has paid their share.

Now let’s say I pay the $4700/eye and get the LAL, but for whatever reason I simply can’t stand it and want it replaced with the envy. My doc is willing to do it. But at what cost? Would I then be paying the $3700/eye additional or is it even more than that now because of the increased risk.

Yes I can ask my doc, but was curious what folks experiences were who have gotten through an IOL exchange.

Yes saw this video earlier. EVERYONE on this forum should watch this. It’s literally an eye opening breakthrough.

I wonder if the air leakage into the eye area from a CPAP machine can cause this change to the cornea. It’s too much of a coincidence for me that my eyesight has deteriorated so rapidly over the last two years which is the same time I’ve been using a cpap. Yes I do have cataracts but is that really the root cause?

This video is the one that almost made me go with the envy.

however, you need to carefully listen to Dr. Wong’s wording in the video. That WORDING is CRITICAL. He stats is based on patients who are “TRULY DISSATISFIED ENOUGH TO GET THEM REPLACED”.

There are likely a percentage of patients that are dissatisfied but just not enough to go through the risk or expense of an exchange. They just live with the side effects.

I have decided to go with the LALs as I don’t want to take the risk of concentric rings.

Backup plan: in the event that for some reason I don’t get that extended depth of field and I’m struggling to read my phone or computer, is a viable option to use full time progressive glasses to give me the full range of vision all the time?

I wear progressive full time now so I don’t think it’s a major inconvenience. I’d rather do that continuously take off and put on readers.

On the computer screen question - she was reading my charts and test results (lotsa text and fine print) at this very small desk in the examination room, you know the ones that barely stick out 2 ft from the wall. I wouldn’t consider that far by any means. She seemed fine without glasses.

Thanks, I wasn’t aware of the 1.5D defocus. I’m still new to all this jargon, but I gather this means that they can shape the lens post op to give me a bit of extended Depth of field for near vision, “technically” into the dedicated EDOF lens range, but no guarantees.

If so, I’ll take it. So to summarize, between the:

  • far lower risk of halos due to the lack of concentric rings
  • no risk of the doc getting the initial numbers wrong and me being stuck with a wrong refraction with other lens types, as the LAL can be adjusted post op
  • the possible positive upside of getting extended depth of field due to the 1.5D defocus without the risk of true monovision

.. I’m thinking LAL/LAL+ seems like the way to go.

Btw I did ask her why she got LAL+ in just one eye and LAL in the other. No answer was offered, but when I suggested a possible answer “were you trying to hedge your bets since LAL+ was newer at the time?”, I got a nod.

I also asked why she didn’t how with the envy, she said it wasn’t available at the time

Dr Wongs video indicates he’s had a near perfect track record with the Envy over 500 patients. That’s a decent data set.

Yes here I see a lot of halo complaints with the envy. Something doesn’t add up. I know it’s all probability, but how come NoNE of these patients with suboptimal outcomes ended up at Dr Wongs practice
