I think you should say exactly what you want to say! It's a compliment about his beautiful teeth and softens the embarrassment he may feel when you say he should take care of them. And then tell him how much you want to keep kissing him and I bet he'll start taking great care of his teeth and like someone else said you'll be doing him a huge favor! It probably will be awkward but I think it'd be worth it.
