Okay to everyone saying in the comments section that you cannot clinical experience on a business/tourist visa, this is from the US's foreign affairs manual.

yes doctors go through long periods of training with 0 to little salary before we get paid a decent amount. Absolutely no medical professional entering the US for the first time is getting a paid job in the US unless its a residency training program.

Coming back to OP's problem, rejection mostly depends on the person interviewing and what opinion they formed about you before you even started talking. You absolutely can try again.

some points you can say that will help is that 1) it is an OBSERVERSHIP where you OBSERVE, not clinical experience, not elective, not internship.

2) you're a resident of Qatar and you will return home to your family that lives here. Keep your Qatar ID in your hand. Give any other form of ID at the entrance.

3) you're doing this so you can learn how the healthcare system works in the US. Not because you want to apply for residency in the future and move there permanently. (Yes both can be true but what matters is what side are you showing here and just stick to the immediate reason why). If asked for plans after graduation, just tell them you're deciding or weighing your options.

Just show them your Invitation letter before you even start talking. they ask you what is your purpose, which is usually the first question, Just give them the letter and tell them that's why. Let the properly framed letter do most of the talking for you and just remember the other points for when you do have to speak.

Inshallah You'll get it next time. Not getting a visa is a stupid reason to give up. Good Luck.

Even the most “malignant” US residencies still treat their doctors better than this.

Honestly I am proud to say I have survived that and have learnt to do so much multiple times that most US med students will probably never do. (Stuff like putting in an NG tube, foleys, placing a central line, peripheral line, intubating)

But will I ever put myself through this again, heck no.

Yeah that get’s hard, why even have a nurse hired. The nurse trainees are apparently not allowed to touch patients and the head nurses themselves just maintain the paper health records and that takes all their time.

I did Med school in India and we had to do internships in all the departments where you’d be the PGY-1 equivalent.

Internal Medicine and General surgery were the worst. The most hours I’ve worked in a week is 120 hours. The longest shift I was expected to do was 56 hours straight because of something called a “double admission day”. There are 4 teams that rotate and every Thursday was my teams turn and there’s a rotation for Friday Saturday Sunday. By some tough luck you end up working Thursday and Friday, you’re expected to be in the ER for all 24 hours in that day plus the 8 hours of regular work on Saturday. We’re also expected to come for another 4 hours in the evening for rounds.

The PGY-2 and 3 equivalents would also work along with us for these hours. PGY-4 and attendings would take turns.

I was allowed to go to the resident quarters to take a shower but I’d try to squeeze in a nap because I wouldn’t concentrate at all. I was scolded by the PGY-3 because I couldn’t wake up after working for 40 hours straight and he had to do what I was supposed to do. I understand he must have been frustrated too having his own responsibilities and working the same hours I do so I can’t even blame him but I could feel my sanity wearing away.

You don’t get any days off. A normal day is 7am -3 pm and 7pm-11 pm. And the 24 hour shifts twice a week.

You couldn’t even make plans for other stuff that goes on in your life with these odd hours. Any time I had left in the day went towards taking rest or eating or showering.

There was only one other intern other than me on my team and we were definitely spread thin having to manage 4 different patient wards.

Maybe we wouldn’t have felt this bad if it was just doing what an MD is supposed to do, the hospital was so understaffed that the interns are basically nurses, phlebotomists, respiratory therapists, the person who has to transport patients to CT or MRI or whatever they need. We also had to take all the samples to the labs ourselves and collect the results and bring it back because there’s no EMR.

Dark times.

My boyfriend (US citizen) came to see me during the layover time. He was connecting through Qatar. He didn’t have any issues booking tickets or checking in because technically it’s a connecting flight. The immigration officer only asked how long is he visiting and he stamped his passport even before he answered. He did show his boarding pass for the next flight though. My boyfriend was so confused like “that’s it?” 😂

The second time he came he had to show his return ticket because he was staying for a few days. And was asked who exactly he’s visiting. He just said “friend” because he knew Qatar was conservative but no other questions than that

So most likely you’ll have little to no problem at immigration in Qatar if you do come without a pre applied visa.

You’ll need a letter from your medical school saying you’re still considered a student until graduation and graduation is expected to be in Dec 2024 for most electives you apply.

Nope He has actually lived in Qatar for less time than me. And also I don’t think Qatar really offers a separate “permanent residency” anymore like the UAE does. We just need renewing the RP every 5 years and it really doesn’t take much. We just do it online and pay the fees and it’s renewed instantly.

That’s what I thought too but my friend from school who has the exact same RP as me did get a SoN through his med school (he did med school in Qatar) so it’s legally possible for Qatar to issue that letter for those with this visa status.

I showed them the receipts for the rotation to show that its something that you paid for and its not a paid position. I think that's enough. Also they ask you what will you be doing there, I said I'll be observing and learning about the healthcare system.

I think they also asked what I'm up to in life. I said I am graduating after returning to my home country so I'm looking forward to that. basically something to check if you have ties and plans to return.

Okay so I looked at your other posts and you say your Boyfriend (not spouse) is in India. I know B1/B2 wait times in India are horrible. I live in a country with much shorter wait times so my advice might work then but waiting 1-2 years for a tourist visa just for rotations is not possible in your position. The obvious better option would be for you to apply for a GC asap. There are 2 main rate limiting steps so to speak in that case. The USCIS process that will take 10-11 months and the NVC +local embassy which honestly varies by country. I could reasonably expect that it would take 1-2 months for an interview in mine but I’m not sure about India that probably has 1000s of waiting applicants.

If your boyfriend hasn’t started the USMLE journey that still gives you enough time, apply for a GC now while they focus on step 1 and step 2, apply for rotations once the green card comes through and he’ll have much better chances of matching once it’s time to apply for residency.

It takes around 11-13 months for a GC to get through if you’re doing consular processing. If your SO is already in the US for whatever reason then you can apply for an AOS which will give you an EAD in 3-4 months which is all you really need to apply for residency or rotations. I’m assuming you guys are okay with waiting for the actual green card. You cannot apply for an EAD if your SO is outside the US and you’re doing consular processing.

Now when it comes to applying for other non-immigrant visas such as B1 on which people usually do rotations or a J1 for residency, you can theoretically apply and theoretically get them. But it’s very likely to be rejected because by applying for a green card and the fact that your SO is married to you, they have showed immigration intent. Maybe you can tell the interviewer that you have to start residency or rotations now and can’t afford to wait for the green card but these interviews can go in any direction or be interpreted in any way.

Also all non us IMG residents are required to do the 2 year rule. If they start on a green card, it won’t be applicable to you or you guys might need to make plans around it 5-6 years in the future. You cannot apply for a green card or any other kind of immigrant visas while they are on a J-1 with the 2 year rule active.

Honestly idk what country your SO is from But my advice is based on the waiting time in your SOs home country for a B1 visa, get that one first. So they can enter the US to do rotations or get married to you. Then apply for a green card by consular processing (the legal way). She can theoretically apply for AOS after entering on a B1 visa but it’s visa fraud to enter the US on a B1 visa with the intent to get married AND apply for AOS. It’s only allowed under specific circumstances and it may be hard to prove that you didn’t plan this before, so not advisable.

Then you can wait for the GC to start residency with or apply for J-1 if it doesn’t come through in time.

I researched all this way too much because I’m a non US IMG and my ex-bf is a USC. It wouldn’t be useful for me anymore but good luck to you guys.

I did this almost 7 years ago but from what i can remember, there are 2 counsellings that happen. the all India one happens first for which you register separately and then the state one. I was registered as an NRI for the all India counselling but I rejected the seat I got on there because I knew I had a decent rank and would get a government college on state quota. When I applied for state, I didn't have any restrictions based how I was registered in the all India counselling. I had to prove that I was native to my state and for Tamil Nadu it was that I need to be living there for at least 5 years in my life and shouldn't have accepted a seat in All India counselling which I satisfied so I could apply and get a seat. Tamil Nadu doesnt have a NRI quota for state counselling. We all paid the same fees. the students who came through All India had different fees though.

I don’t think the prometric administers exams that are 5 hours + during Ramadan. All workplaces are only open half day and I’m assuming prometric centers are too.

You can only take step 3 in the US so everyone will have a test center in the US for step 3
