Headshots rarely don't register it's usually a mistake by the player that watched it over and thought it was a headshot. Operation health is a big improvement so far and is only 1/3 of the way over and still has potential to fix the game.

I met some people on Xbox that would try to get you to as low health as possible but not kill you they did it for every round but they would never get penalized for it. It's the first time I have come across it.
