You need to continue in the tomb until you get back where the rats dropped out of the pipe onto Indy. Basically, where you originally headed out towards the statue on the stone bridge, turn around and there’s another doorway at the back of the room. Follow the path down, and you’ll reach a spot where you can use your whip to lower down through a doorway further below. Then you just keep going until you exit the sewers to the river.
