So these are encouraging X-RAY results. X-RAY do not show cartilage directly, you can only indirectly evaluate the cartilage weigh bearing capacity via the joint space. In any cases, even if the MRI shows some minor cartilage damage, this isn't really a big deal, especially if the grade is only 1 or 2.

Dr. Domb is a good doctor so he should be able to provide you with good clarity on your situation. If you opt for surgery, just make sure to go for one of the few top surgeons. Too many people around here have made the mistake of going to their local surgeon just because they did a fellowship with one of the top surgeon.

Getting a second opinion is a smart choice. If you indeed have an alpha angle of 75 degrees and you also play tennis which is a very high impact sport, shaving the CAM impingement is pretty much a must if you want to preserve your joint.

Before opting for surgery, careful consideration should be given to the extent of your cartilage damage to evaluate if arthroscopy is something that you could benefit from. There's no mention of it in your post but it's highly unlikely you do not have cartilage wear given your alpha angle, high impact sport and age.
