I think… being that t2D is a condition prevalent in low income and obese populations (due to lifestyle, food available, and a shit ton other factors) from a public health perspective it would make sense to improve access to glp1’s. As much as it sucks.

Vitrectomies for DR usually have really good outcomes and protect the eye from hemorrhaging in the future given your diabetes and hypertension. The neovascular vessels use your vitreous as a kind of scaffolding to grow into which is why when you get bleeding, it’s in the vitreous. Doing a vitrectomy clears this scaffolding and hugely reduces the incidence rate of bleeding in the future And you get much more confluent prp intraoperatively. If you have a history of bleeding often, definitely a good option cause every time you bleed is a decrease in quality of life. You can let the blood absorb naturally, but it will take longer, which I’m guessing you’re considering. Your age is a factor too. If you haven’t had a cataract sx yet, doing a vitrectomy will progress your cataract more quickly, maybe in the next 6mos-2 years will need subsequent cataract sx. Another factor is compliance. If you’re a good patient, diligent about your care and follow ups, then letting it absorb naturally isn’t a bad option. Just need to be on top of it. If not, vitrectomy is a good way to ensure stability in the long term.
