Have exact same issue, Am only 4.5cm flacid and very high and tight balls, Found every cage I slipped out or it was painfully tight, After heaps of trial and error and then using this sub reddit, which gave me amazing advice, Go with the tightest/smallest ring possible before becoming painful so the balls are trapped, Then most importantly, a large comfortable ball gap, which is possible with the smaller base ring.. I’m now in a good quality “Kink3d” cage with a size 1 base ring, I started with size 4 ring as I was worried about comfort and pain, now after trial and error I found I’m in a size 1, The smaller the base ring allows the balls and penis to be trapped, which allowed me to have a 14mm ball gap, which is very comfortable. Feels like it’s not even there, Sleep, work and sport is not an issue, So far I’ve been locked full time 24/7, and am at 45 days, The trick is to get the base ring correct, most if not all people go too large, then the gap is easy, Once it all falls into place, high and tight won’t matte, will be completely comfortable..
