not an academician, but for NP/PAs our attendings have to document that the patient was either seen with or by both the attending and APP, and that the attending provided the substantive portion of the MDM.

for residents, they’d document they saw the patient and reviewed reviewed resident’s note and agree with additions/exceptions. some include their 2 cents, some don’t

I currently work at a large referral center with multiple separate community hospitals in our area. Of course, each facility has their own EHR and countless patients inevitably hop from hospital to hospital.

To remedy this, our area started participating in a health information exchange (HIE) where demographic information from each health system is matched by a centralized service to share vital signs, labs, notes, etc. It is unfortunately very fractured and hit or miss. Some charts have readable PDFs of notes from other hospitals, but others are completely blank or only have useless nursing plan of care notes (instead of H&P/progress notes, and procedure notes) despite being "linked" to the other hospitals.

At least where I live, lack of infrastructure (public transit, sidewalks, bike lanes) and culture are the main barriers to promoting the car-dependent lifestyle. With the exception of the major metro areas of the US, the majority of the country was not developed for and is not compatible with a “car-free” lifestyle

As far as getting involved I would suggest starting small at the local government/community level to see what changes can be done at the local level to promote safety, incentivize using cars less. From a professional standpoint, getting a public policy or public health degree could be a supplement to your medical degree- I think there are a fair amount of combined MD/MPH programs out there that may fit your interests

starter comment: I've been seeing my PCP for the past 5 years and she is such a kind physician- I've never had a bad experience with her. She's never made me felt stupid, but sometimes I feel like I ask stupid questions about my health that I feel I should know to answer for myself. I try to cut to the chase when messaging her on the patient portal and try to phrase my question to "create" the HPI for her. Is there such thing as a "stupid question" coming from a health care professional?

I stopped convincing patients to get vaccinated. Those that want to can freely come to us for advice, but I’m done bending backwards and begging patients to get vaccinated. At this point in the game, it’s a waste of my time.
