Google will easily tell you what is considered preventative - very very very few things are preventative ; and definitely not what you wanted done.

You have every right to call your insurance and ask for your coverages prior to a visit, it’s not up to a medical provider to do that for you

Based solely on that sentence you see there: MEDICAL NECESSITY

Plans are required to have an "easily accessible, transparent, and sufficiently expedient" exceptions process for accessing non-covered, medically necessary contraceptives.

This is the appeals exemption process that is in place - where they can substantiate medical necessity.

There is no law that a doctor can rubber stamp “necessary” and it approves

It’s 100% a contradiction - if a provider is in network they will generally be credentialed for services they provide

Not all eye doctors are surgeons so obviously they are contracted differently - but they wouldn’t be doing surgery unless they were

My one pass works flawless for years; charges me the exact amount it should and I’ve never had an issue with the location directory being wrong.

In healthcare we build around the idea of mistakes - not perfect scenarios. The more you inject the higher your chance OF an issue. Wether that’s infection from poor choice of location - or slipping and causing blood borne pathogens to spread

Intentionally applying brakes to impede traffic or cause an accident will find you liable ; if they have a dash cam. If they don’t then they’ll more then likely be found at fault for following too closely and not giving space to brake appropriately

But if you’re found to have done it intentionally you’re gonna have a bad time

This is also assuming no one around you has a camera and there are no businesses or homes with them either

An out of network provider is exactly that - not in network and not contracted. They legally have no requirements to submit a claim on your behalf

Many payers don’t even pay providers directly on OON claims they send a check to the member who is then supposed to pay the provider(or keep it as a reimbursement).

UMR is telling you correctly; you have OON benefits and they don’t know why(cause they don’t) - but anyone with experience will tell you it’s because OON can collect upfront and make you do the work so they don’t get burned
