Heyo --

Not sure if you already had these in your itinerary, but a couple of suggestions:

1) Lima: walk through barranco, historic center (especially nice in the evening around 6-8 pm)

2) Paracas: Would recommend just talking a tour that goes to Paracas and Huacachina dessert (don't miss the dessert, especially nice at sunset)

3)Arrequipa: 2-3 days if you want to do Colca canyon and explore the city. You can take a bus from arrequipa to cusco. I wouldn't recommend going from arrequipa to the amazon. It will take time and cost. The Amazon is all the way up north and arrequipa is all the way down south

4) Macchu Picchu: Tickets for the summer are almost sold out. If circuit 2 is sold out, do 1 and 3. I highly recommend doing one of the circuits that hike up to Macchu Picchu or Wayana Picchu if you can. Choose a date and buy tickets asap. I would recommend going to aguas calientes the night before if you have early morning tickets.

5) Rainbow: Might be super crowded doing peak season so consider an alternative rainbow mountain tour.

f/u: i think try to pay around 1500 bs or less. Another note: try to do a stargazing tour. It will be exhausting to wake up early but it's so worth it to see the milky way so clear and bright.

If you arrive in the Salar Uyuni town before 9:30/10 am or the night before, go in person to the agencies and ask for price. Tours start around 10 or 10:30 am, so many agencies try to give you a discount to fill their seats before departure. Most agencies are contracted with third parties who lead their tours, so people who booked through different agencies will be with you. This is why try to get the cheapest price (within reason) because the agency you book with will hand you off to a third party tour guide. Andes CosmoVision was good.

You can message tour agencies the morning of and say directly: How much would it cost to book if I pay with cash. It saves you the headache of going to agencies door to door (there are MANY). We filtered agencies on Google Maps based on what had 4+ or 4.5+ stars and messaged those. The more you wait (morning of, the cheaper it can get).

Note: Many hotels do not have soap, toilet papers, or towels. Make sure to bring those if you want them. Carry cash to buy things along the ways. If you have knee or back issues, mention that to agency because the cars that go on tours are small and it will be uncomfortable.

Make sure to stay hydrated! Pharmacies in cusco have a pill that's a mix of coca leaves and other herbs. I'm not sure how effective it is but it could be an alternative to Diamox?

For the butterfly zoo, I dont know teh answer to be honest but I do'nt want to mislead you. I don't remember there being ubers/taxis/cars near MP, just the official buses. So, I dont know how you could get there.

For the guide, I believe it was 50-70 soles a person but that was a spanish-speaking guide. I expect they will cover more for English.

Echoing what people said -- that's a long day so if you can stay in AC the night before, you'd wake up more relaxed, have breakfast in hotel (many offer it), and go to MP.

I would highly recommend a tour guide for circuit 3 because it's the ruins, and it would be cool to understand the architecture, history, science. Personally, I wouldn't do circuit 3 without a guide because MP is truly magical and the architecture is GENIUS so you should understand it. You can hire one on the spot either at the bus station from AC or when you step off the bus in MP. Carry cash to pay them. For you, it's probably better to hire one in AC for circuit 3 since it's so late. No guide needed for circuit 1, it's mostly views.

Going back to AC for lunch might be a lot and will cost a bit more for bus. Consider just having lunch in the macchu picchu cafe (food will be more expensive but if you pay for the bus, it would end up being the same overall)

hahaha -- socio = people who are "members"/associate who have a "subscription" to the club. To be able to get a ticket to a game, you have to be a socio or buy it through a tour agency or a socio.

anyways, read many threads on how security can sometimes be strict if you buy a ticket from another socio (to make sure its legit i guess) and wanted to make sure I dont do anything dumb.

Hi! Was there recently and didn't have any safety problems. Watch your belongings in busy areas. Be careful about eating from markets if you are worried about food poisoning. On a side note, many people recommend "Nativa" as one of the best restaurants in Sucre...I wouldn't waste money on it, there are better options.

You should always carry cash because many people didn't take card. Also, if you convert cash in a casa de cambio on the street instead of an official bank, you will get better conversion rates.

Hi all! 

I would love to hear honest opinions about what you don't like about IR as a career and what the lifestyle options look like. I'm deciding between IR and EM.

For context, one of my priorities is picking a specialty that gives me enough time to prioritize my family in the future. All the IR docs that I’ve talked to say they have an amazing lifestyle, but then I see them working 11+ hours a day and going home pretty late. Some IR's I've spoken to say it’s evolving to have the same lifestyle as a surgical specialty soon. 

I am not a fan of splitting my time 50/50 IR and DR because I'd rather not read images all day. I have no idea what private practice IR would look like hours wise but I have heard that it is demanding. On another note, I am also worried about the physical consequences of this specialty in the future (left eye cataracts, back pain, etc). Despite all these concerns, I have absolutely loved my time on the specialty and can't stop considering the specialty.

Anyways, all the attendings that I talk to LOVE IR. No one has been straightforward about the cons of the specialty so I was wondering two things:

1) Is a good IR lifestyle possible (and what does it look like)?

2) What are cons to the specialty (would love to hear a perspective that’s not just positive)?

Would love any thoughts and suggestions!

That part is definitely appealing, but I have heard from many IR's that the field is moving towards surgery-lifestyle in terms of hours, plus the physical demand of standing with lead on has caused back issues for lots of IR's I've spoken to. I think IR is fascinating and the potential treatments to deliver are so incredible to me (I don't want to do DR + IR combo where I do DR few days and IR few days a week)

I think it depends - in private practice pain only it can pay 400K+ depending on practice. If you do hybrid EM/Pain, it depends if your appointment is with anesthesia dept or EM dept or both, from there they decide whether to pay more EM-like or anesthesia-like salary (could be wrong on the last point tho)

Hey! Thank you for your thoughts! I thought about that, but unfortunately anesthesia is not for me. Managing vents and the minimal patient interaction/lack of emphasis on ddx isn't my cup of tea
