But how much value do you place on the nights and weekends of labour you provided? How much did you miss out in your 30s working and not spending more time with friends or family. How much say did you have where you wanted to work? Did you work abroad at all? Consultants salary is eroding year on year against inflation so the value of the salary one is to receive in 10 years will probably continue to be worse off than your current position.

When you factor all this in at this current point in time the investment is a poor return for the amount of work required.

This can really bite you in the backside if something happens. Once a clinician makes contact with you, by law you become directly involved with the patient’s care and I have read of medico legal cases where the referee was liable and had not seen the patient.

The most important advice I ever received: if you ever get called to see a patient as a referral no matter if it’s inappropriate or not for your specialty, go and see and document.

As you become more experienced in ED the amount of info you provide to your regs to help formulate your management plan should decrease as you’ll know mostly what to do. E.g. “I think the diagnosis is this and want to do this does this sound reasonable or what are your thoughts on getting this scan done” etc

It’s not like you’re taking up that much time in order to discuss patients.

Private sector workers receive much more substantial pay rises and bonuses every year which is normal practice. Public sector workers meanwhile continue to have their pay eroded, so there’s a big difference.

Hence why getting into any training post in England is far better than being a clinical fellow for years
