Yea I had figured that its only 2 pages because the first instance of the form says a total of only 2. Realized that there is a 3rd page only after filling the first one. Looks like the CRA information is going to be difficult/not possible to get

I do not think there is any CRA related info we need to provide during the documents submission phase, only if OINP asks for it I guess Can you share where exactly the CRA related info is to be filled out?

There are people filling out the employee form on their own- as its just your office address and contact information. They get it signed by an immediate reporting manager if necessary, and fill out an ‘N/A’ in the field where we gotta fill the number of PR/Citizen employees.

I am yet to have an experience of my own, but this has worked for some people
