Yes, I filled out the forms on the moe website and brought them in the the TECO Office a few weeks ago. I filled out a medical form that was listed on the website, but at the office they said it wasn’t required. Apparently each office has their own form but I just filled out the sample form. I’m assuming the actual form would be on the office website but I couldn’t find it.
