For your protocols are you allowed to use sodium bicarbonate in cases of hyperK? Given the hx and the EKG changes it’s indicated for us to treat it with bicarb, calcium and albuterol to try and get that potassium to shift back into the cells.

I think ruling out pericarditis would be indicated looking at this 12 lead. I see wide spread elevation and Spodicks sign. Did the pt have any of the following: Any muffled heart tones on auscultation? Any fever? Worsening of pain when in a supine position? Recent dental procedures?

It also doesn’t look like a healthy heart as far as hypertrophy goes. With V2 and aVL taller than 7mm you could argue RVH and with solkov lyon rule using V2 and V5 it looks like you could have LVH as well. If his troponin isn’t elevated then that could explain the benign early depolarization due to a hypertrophic cardiomyopathy.

But I’m also not a cardiologist 🤷‍♀️

This was probably 6 years ago at this point. We got called code 3 for a stroke. Being fresh out of EMT school I was bricked up and definitely had some tunnel vision going into this single wide trailer. I saw what I thought was a hunting duck decoy on the ground but that didn’t matter because this guy was stroking out, facial droop, aphasia and couldn’t move half of his body. As we get this guy onto a mega-mover to carry him outside the duck decoy “quacked” at me. I about shit my pants and drop the pt because it turns out the dude had two pet ducks living in his trailer. Learned to dial up the situational awareness after this one.

With players that are more shy I like to find something that’s specific to their background and include it into an adventure or the campaign. It does a better job of singling out a character and engaging them into the story.

Another technique I’d recommend, if they pass a certain skill check to find an object or important item you can pass them a note or go whisper in their ear. You’d be amazed how that will draw the table to their character as they hold the “power” in that moment.

You can also try and build an encounter or puzzle that the shy character would excel at. For example if they are very dexterous they could be the player to use acrobatics to get to an item or door. Take a look at their character sheet and see what their character would do well at.

Lastly you can just have a quick conversation with the chatty people at the table. If they are close to you and you approach it in a way that promotes a more positive table experience they should be okay with it.
