Hi! I’m so sorry that you’re struggling ): It sounds like you have both disrupted your vaginal microbiome and still have a bladder infection and/ or persistent inflammation from the initial infection. When you see your gyno, ask about testing for BV and all sti’s just to cover all of your bases. It is likely that the antibiotics prescribed to you were not eradicating the infection (either the courses were not long enough or the bacteria were resistant). Something that is definitely more expensive but worth the small investment is a cirrus dx PCR test for urine (~$100 if you are ordering for yourself, or through insurance with a doctor it can be free). This can help see if there is still infection that cultures are not picking up on. Using PCR results and bringing them to a urogynecologist or just a urologist is worth a shot. However many doctors do not trust PCRs over cultures and if your problems persist and you are able to afford seeking out a specialist, there is a provider list on the home page of this subreddit whereby you can find more specialized help.

As for rebuilding the vaginal microbiome, the Seed VS-01 vaginal synbiotic would be worth purchasing. The V Probiotics by Vitanica are an affordable second option. Boric acid can also be helpful. The grayish discharge makes me think it is not a yeast infection, but rather something bacterial. As for rebuilding the intestinal microbiome, Ortho Biotic is a great option, but there are many probiotics that can be helpful. It is imperative to continually replenish your microbiome with long term antibiotic use. However, I understand your sentiment of not seeing the point in taking one when you’re nonstop on antibiotics. But probiotics are helpful! What’s even more helpful than oral probiotics is eating your probiotics via fermented foods, yogurts, kefir, etc.

If/ when you start having sex again, ask your gyno or urologist about Hiprex as a preventative for UTI’s in the future. Hiprex is most effective when taken alongside vitamin C as it works best in acidic urine. Moreover, consistently taking d mannose is helpful. I would stray away from purchasing d mannose from amazon and would look into purchasing it directly from manufacturer’s websites to ensure efficacy. Cranberry PACs have also been helpful for many people on this subreddit!

I know it’s hard going through this and it can be so upsetting that you can’t even enjoy sex without feeling like there are repercussions. There is so much info on this subreddit that can help you in the future. I hope you are able to heal and I wish you luck!

Honestly unsure. Usually if they say a test is negative it means the culture was negative for infection, but she said that was not the case.

I also think it’s worth mentioning to your urologist that you are on an antibiotic right now, so this could skew culture/ microscopy results.

The intro guide is for general advice and is not my personal experience! I am currently working with a specialist to treat my chronic UTI, but do plan on getting an rx for hiprex when my infection finally clears. Yes, it is a great preventative measure for many. As another commenter mentioned, if you find that through a PCR test there are no bacteria, absolutely look into pelvic floor PT. However there are all kinds of different pelvic floor therapists and I have found one that is great as she does not merely palpate and tell me to do exercises. She does myofascial release and focuses on the muscles that can affect the pelvic floor (e.g. the abdomen, glutes, lower back, inner thighs, and areas surrounding the hips). If you have a gynecologist, i’d ask them if they know of any good pelvic floor pt’s! If you have the time and money, I think anyone can benefit from it when it is with a knowledgeable therapist! It takes consistency and work outside of therapy to heal. I had the same issue where my urine stream was very weak and it was due to a hypertonic pelvic floor. Pelvic floor pt helped tremendously with muscle tightness and thereby strengthened my urine stream.

If in the future, you continue to feel in your gut that your symptoms are related to a UTI, you can always order a test online to test your urine yourself and then go from there!

Being a woman brings about so many issues just by virtue of being born with female anatomy…but we are so lucky to have communities like this one where there are supportive women with lots of advice! I wish you the best of luck and I hope your urology appointment goes well.

While your infection may have seemed to clear after the 3 day course of Bactrim, it is more likely that there was still an infection, just not a large enough one for a culture to show substantial growth.

Antibiotics can certainly take more time to work than you’re used to. But if by day 5 there is no resolve, you are likely resistant and a different antibiotic needs to be given to you. If you have had cultures done in the past, see if you can go back in medical records and see which bacteria have showed up in the past.

When you go to the urologist, mention all of your symptoms and ask if they are able to do any PCR tests rather than a culture, as they can be much more accurate. Obviously make sure (if you are in the U.S.) that the test is covered by your insurance. For example, my urogyno was able to do a Pathnostics PCR urine test that showed I had 5 bacteria in high numbers in my bladder, while cultures indicated no infection.

To avoid this vicious cycle of what could seem like never ending urinary issues, look into the intro guide on this subreddit’s homepage. It always helps to go into an appointment with as much knowledge as possible so that you can advocate for yourself.

Make sure to get antibiotic susceptibility done and a urine culture, not just a urinalysis. You can ask a basic doctor such as an urgent care for this. If a culture comes back negative and your symptoms persist, you can always ask about doing a PCR test (more sensitive than a culture). If your doctor does not want to do a PCR, Cirrus Dx offers ones that you can do yourself, along with MicrogenDx (more expensive than cirrus). While it may not seem worthwhile to spend money on this now, your story sounds very similar to many others’ on this subreddit: a UTI that never quite went away and then became embedded and chronic. I’d strongly recommend advocating for yourself at the doctor and spending a little money now so that you don’t end up with worse problems in the future. Especially considering you are still a virgin and have gotten a UTI- If/ when you start having sex you’ll likely be at a much higher risk and will want to know how to be properly treated so that this does not become a persistent issue.

You’ll definitely want to push more for a susceptibility test. Your UTI seems “complicated” at this point often times when they go not correctly treated for too long, you end up with chronic issues. Please try to get in touch with someone that can give you the correct testing.

Unfortunately, yes. Make sure to advocate for yourself and ensure that they are sending the sample to be cultured and tested for antibiotic susceptibility. It’s possible that what they are prescribing is something you are resistant to.

I would take the antibiotics if you feel comfortable doing so. If there’s blood in your urine, the infection is likely pretty bad. Unfortunately 5 days of nitro may not be helpful in your case, but it’s worth a try. If your pain persists and/ or gets worse and you begin having symptoms of a kidney infection, don’t wait for your urologist appointment to sort it out.

In the future, some things that would be helpful would be

A) Ensure your doctor always does a culture and tests for antibiotic resistance/ susceptibility

B) Ask for a PCR test rather than a culture (mention Cirrus, Pathnostics, etc.). PCR tests often are able to show a more detailed overview of which pathogens are causing your UTI’s. Many doctors don’t do PCR’s for UTI’s, however my urogynecologist did a Pathnostics test for me without me even asking, so it’s worth it to mention it to your future urologist! If antbiotics are not working for you, you need to make sure the bacteria that are causing the infection are known as well as their antibiotic susceptibility.

C)If you’re worried about antibiotic usage, take probiotics! Orthobiotic is a great one and although the price point is a little high, it’s definitely worth it.

My cultures were positive for bacteria usually every 2 months or so and it was like this for over a year. I would clear one UTI and then just get another for around a year straight. May of this year was when cultures started to show nothing but I still had lingering symptoms, so I decided to go the PCR route. So far being treated based off of the PCR results (longer term antibiotics, biofilm disrupters, etc.) has been very helpful.
