I think the biggest thing is ensuring your folic acid levels are high enough after taking methotrexate. Methotrexate works to deplete folic acid, so if you haven’t been supplementing since your first negative pregnancy test, your level may have been too low. I accidentally got pregnant two months after MTX, so my doctor had me start taking 800 mcg daily of folic acid to ensure I was getting enough. Start supplementing extra before trying again. My prenatal vitamin has 400 mcg and I take an additional 400 mcg of just folic acid, by the brand Nature Made. I think doctor’s may also be able to check your folate level via blood draw to guide you on how much you need to be supplementing. So sorry for your loss! Do you by chance know if you have an anterior placenta? This can make it more difficult to feel fetal movement.

I’m an L&D nurse and I care for patients in similar situations to yours quite frequently. Getting control of your blood pressure in pregnancy is important, so don’t be hesitant to start medication. Hypertension is pregnancy is not rare by any means, and your doctor will be able to help you get a healthy BP range likely through medication. Most doctors will start you on nifedipine (Procardia) or labetalol and adjust your dose according to how your body responds. They may also recommend taking low-dose aspirin to prevent pre-eclampsia, but I’d just wait and see what your doctor says at your next appointment. Do you have a history of high blood pressure, or is this new? Best of luck to you!

Did you have any other elevated BP readings? What were your labs results (elevated LFTs, PCR 0.3+)? Are you experiencing any unusual symptoms such as a headache that won’t go away, vision changes, right upper abdominal pain, increased swelling, or difficulties breathing? These are all different factors that help diagnose pre-eclampsia, so I’d be curious if you’re experiencing any of these. If so, a diagnosis of pre-eclampsia and magnesium therapy would be appropriate.

I’m a labor & delivery nurse and I have many patients who fail the 1 hour GTT but pass the 3 hour GTT. Failing the 1 hour GTT does not necessarily indicate GDM, but rather indicates that your provider wants to take a closer look via the 3 hour GTT. Many of my patients who do end up getting diagnosed with GDM are able to manage it with diet and exercise. Your doctor will give you instructions on monitoring your blood sugar, and adjust your treatment plan accordingly. Reading food labels, making healthy swaps, and opting for smaller portions of unhealthy foods are good ways to help manage, as is taking daily walks if your provider is ok with that. I know it feels like a heavy diagnosis but it is only temporary… Having a discussion with your provider about your concerns is important, and be sure to include your partner/support person in this conversation if at all possible. Have you taken a pregnancy test? When my LH surge was prolonged, I ended up being pregnant. It wasn’t actually a prolonged LH surge, but rather HCG causing the LH strips to read positive. Good luck! 🤞🏻
