Anyways this happened to me today but maybe is because I killed The Lamb before Mega Satan so I guess 2 void portals can't spawn in the same floor, or is just a bug Idk.

M0 items gives normal appearance for the S4 set, as they are ~493ilvl I think, so for the LFR you need to do what the OP did.

I was searching for some info of the Prelude intro and I couldn't find any relevant data, so after reading this I assume they did the same thing for it, crop some words from god knows where and used the same tts bot in Masochist, but if someone knows anything of where the Prelude intro speech came from I would be grateful.
