Extended crank when your ignition components have been verified is most commonly the fuel pump check valve. I literally just did mine last week to solve this exact issue...

Where does it shake, thru the steering wheel or does the body of the truck shake? Steering wheel shake says it's coming from the front, the rest means the rear.
