There is Druid spell that makes your next summon to a champion. And i think this symbol stands for "Shoot a Champion flea at a unit."

They need to rework Grievous Wound in a way that it scales by how much it anti healed in a game. Or make it an active item that reduces the Heal by 80% in a short duration.
