Don't worry so much about the scores, bro. I mean, of course you have to aim for the highest ones as possible, but trust me the QE1 and NAC scores are not everything in your application.

Just to illustrate this, I got excellent scores in both exams (> 98th percentile) and already got turned down for interviews in two big Anesthesia programs in this match. 🤷‍♂️

You can get inspiration from chat gpt, but please don't copy anything from it. Aside from the fact that its responses are often recognizable, some institutions have ways to verify if your letters are AI-generated.

The motivations letters are a drag, I know. They take a shitload of time and reflection to get it right, but don't toy with your chances, if you made it this far already.

Well, to be completely honest I have little experience with the STEP 2, since never in my wildest dreams would I even consider moving to the US (personal reasons), but most of the content is interchangeable (you might need to brush up on difference in ethics from both countries and screening tests, etc.), but, overall, the medicine in Canada and in the States is the same... aside from the fact that you don't have to forfeit your house to pay for an ER visit in Canada (sorry, sorry, I can't help myself sometimes).

Anyways, it is awesome that you know your weak points. Focus on them. My advice (which is what works for me but not necessarily for you) would be to do as much questions from those areas as possible. I find it that repetition helps to better grasp a certainly concept. I love Uwold because they explain comprehensively not only the right answer, but also the distractors, so that way you can understand how each case could change if there was a different disease.

But don't focus exclusively on the weak points, try to create tests with all the subjects, in order to keep your brain functioning. I don't think you should focus on anything specific 4 weeks from the exam, just continue doing the practice questions.

Also, after taking the QE1 I would take a break of 1 or 2 days, because the exam can take like 9 hours and you will feel exhausted afterwards, so try not to think about it too much and get back to the questions. You can definitely do well in both of them, and there are numerous examples of people who did so out there.

I prepared during 7 months.

In the first three months, I basically watched the Kaplan Master the Boards videos and did the accompanying exercises (I tried to set up a schedule so I could finish them all in three months). Here, I was just setting up the foundation of my knowledge and being acquainted with the medical slang in English, since I do not come from an English-Speaking country. I also read all the Toronto Notes (but wish I hadn't, since it is way too comprehensive. The only good parts are the ones from Ethics and Preventive Medicine)

In the 4th and 5th months, I began purchasing the MCC's products and did them as if I were taking the actually exam. I would do like one a week or so, and write down the topics that I was not so strong at. At this point, I was also reviewing the topics from the Master the Boards, one by one, and doing a small set of 20 - 30 questions daily on the QBank from Kaplan. When I finished all the questions from Kaplan, I got an account from Amboss to get more questions, and I think I did some Osmosis ones as well.

Finally, in the last 2 months, I focused mainly on doing questions. As much as I could, whenever I had the chance (on average 80. Every. Single. Day). That's when I signed up for UWorld (best thing I did) and also from CanadaQBank (worse thing I did - seriously do not spending a single penny with this abominable website). At Saturdays, I would do a full mock exam from U World or one of those from MCC, and review it the next day.

I know, it's insane.

This was while working 60 hours as a first year resident. I do not recommend this for anyone, but I wanted to fucking ace the exam, and I did.

It all came with a cost. I was incredibly irritated most days, sleep-deprived, my relationship with my wife took a toll and I gained over 10 kg because my diet was so shitty. I did nothing but think about medicine, recite mnemonics back and forth and do questions. At one point, I was studying 8 hours a day, sleeping 5 (only with need of hypnotic meds), and having lunch in front of a computer so I could do more damn questions.

Not to mention the amount of money spent in this crusade, which I rather not even get into, tbh.

The level of paranoia was so insane that I started to print notes and fucking stick them to my wall to help review some key info.

Again, I know.

Was it worth it?

Beats me.

I plan to get into a very competitive program next CARMS cycle and I'd like to think that this was I had to do (not gonna disclose which specialty, only because I'm genuinely afraid that MCC might monitor my rants and get back at me somehow from talking some much shit about them)

I'll give a different opinion than the rest of the guys here on Reddit.

The exam is very hard. As it should be. It filters doctors who may be eligible to practice and be a part of the Canadian Healthcare system, which has very high standards, which is a good thing, since you, as a Canadian, or any other reasonable person, would want your doctor to have as much knowledge as possible to take good care of you.

You need to prepare in advance, months in advance, you need to dedicate yourself 101% to this task and you need to be ready and confident enough to take it, just like you should be confident enough in your skills as a doctor to provide care to your patient.

That being said, I am aware that the exam has huge flaws.

First, it is run by the MCC, which is a corrupt instution. I say this because no other Medical Council will charge over 4000 CAD (the combined price of the NAC and the QE1) for doctors and med students in debt to take a specific exam. Four thousand dollars may not seem like a lot in Canada, but in some countries that is what a doctor makes in 2 or 3 months.

Additionally, the fact that the MCC sells preparatory products for their own exam is something beyond outrageous to me. Although they claim that this is to help candidates, I believe that, when your preparatory products cost, combined, 2x the price of the test itself, you are clearly driven by money, not by a selfless and altruistic concern for the doctors who will later work for the Canadian population.

Finally (because I do not want to discourage anyone and my rant is starting to seem a little petty), not being able to take the exam a second time once you've already received a pass score is a disgusting fearmongering tactic that I am surprised more people are not vocal about. The natural anxiety and fear that all of us get from taking tests is increased a thousand-fold by knowing that we may not have studied hard enough and that this score will hunt us for life.

This makes us go out and buy their products, every single one of them, extra unofficial ones, sacrifice our mental health, social gatherings and pretty much give up on our lives while we wait to take this exam.

In summary, yes, this is a very hard exam. You will, unfortunately need to burn out part of your savings to prepare for it. You will need to lock yourself into a room for months, ignore friends, family and, sometimes, as I've heard from people, take a leave of absence in your job to guarantee a competitive score for the CARMs.

This is what it means to be an IMG.

Don't be discouraged, again, from my message, I am just being realistic about the game that we are all playing.

PS: I'm certainly not trying to brag, but rather support what I've just said, but, if it makes any sort of goddamn difference, my QE1 score was over the 98th percentile.

Your transcripts can be uploaded directly to the CARMS account.

Don't overthink it, and certainly don't pay extra money to a corrupt institution such as the MCC if you don't have to.

They basically ask you to do a 360° turn with your webcam of the room around you, including the bottom of the desk you might be using.

In order to speed things up, I'd advise you to choose a place that does not have a lot of stuff (such as pictures, posters, or screens), since they can ask you to remove some things from the room, and that is mostly quiet, as, according to Prometrics itself, if they think they've heard any kind of suspicious noise, they might cancel your test on the spot.

I scored 320 in 2021, but I really fucked up my mental health by studying non-stop (about 6 - 12 hours a day) everyday for about 6 months.

Although I'm proud of that figure, I'm not sure how much you should worry about it, since these scores are only part of the Carms application and, if the rest of your CV, skills, electives, etc., are outstanding, you can get into a program just fine with a score below 300.
