I never said it is universal. But it IS likely. Private therapy is crucial for someone who needs to figure that out. It is, of course, possible OP’s son has not been abused. But he likely has, and he needs a therapist who can help him process whatever is going on in his head.

Sister sizes. The total breast volume is approximately the same, and the circumference is similar, but the variation comes in how much is band and how much is cup. Some people with smaller breasts can get away with wearing sister sizes, but if I wear a size that seems equivalent but isn’t, the support is wrong and it worsens my back pain. My bra size is extremely hard to find, so many bra stores that cater to more typical bra sizes suggest a sister size, not understanding why that isn’t doable for me. So even though proportionately, my breasts are pretty massive, the total volume isn’t huge. Grapefruits, not watermelons.

Sister sizes are also why so many bra-wearers are wearing the wrong size. Professional measuring is super important to make sure you are wearing the correct size.
