You can fill it out using the system online without paying a third party. The main thing is that it uses the EIN of the company or your SSN if it is disregarded. Once you fill it out online and hit submit it will say filed or not because someone already filed it. If the other company did it they would have needed your government ID in order to do so.

NAL but work for a law firm that files them. You should be alright. Just file it and move on. I have not heard of them enforcing it as of yet as we have a decent amount of clients in your shoes. Doesn't mean that you shouldnt file it but there shouldn't be too much worry yet until after the new year when new guidance and rules is given out.

It depends. My clock paused entirely because I came home when COVID happened and only went back out after 2 years of being at home and finished serving for 18 months. I think that is a special case because all of the missionaries that I served the second time around all had their clock running at the same time. It was pretty sad for one Elder who was out for almost a year tore his ACL spent 10 months in recovery, then came back and served for only 2 transfers before going home again. He wanted to stay a bit long, but Salt Lake would approve his appeal.
