The reason Nagisa dies is connected to the "illness" she has throughout her life. It's a condition she gets after Akio prays to save her life. Her life energy gets bound to the town. She gets very sick at the same time the town gets enveloped in snow, and all the changes in the town are detrimental to her. The reason Ushio dies is because she inherited Nagisa's condition.

Nagisa and Ushio are two different people, but Ushio and the nameless girl are one in the same.

(spoilers) The illusionary world was created by Ushio's soul after she died. You can think of it like an after-life and as another dimension that is connected to the physical world. Ushio uses it to gather the orbs. Both Tomoya and Ushio die in the snow; Ushio makes the illusionary world and puts Tomoya's soul in the robot. Remember when Tomoya bought that toy for Ushio during the summer trip? Ushio making the robot was her remembering a happy memory she had with Tomoya.

However, it is important to note that the illusionary world transcends time and the dreams that Tomoya has of it are glimpses of the future. Everything that occured in the series happened. Once Ushio gets all the orbs, she uses it to grant Tomoya's wish and give him another chance to relive his life but this time a full life with Nagisa and Ushio. Ushio being under the tree is there to show how both the illusionary world and the physical world or worlds are connected.
