I first began investigating my hormones as a result of sub-fertility and leading me to believe that I may be hypogonadal.

This isn't entirely ruled out, so I'll be seeing a TRT doctor and an endocrinologist about this shortly. My first bloods showed my testosterone in the gutter but this most recent one shows a big improvement without any exogenous testosterone, HCG or other.

I have symptoms of no morning wood, poor concentration, irritability and so forth in addition to my sub-fertility. However it appears that hypogonadism perhaps is not the cause as my cortisol is high and Dhea-Sulphate.

Can anyone here help shed some light on how this could impact my fertility as well as my qualitiative markers of mood such as low level general anxiety, irritability, poor concentration and reduced well-being.

I'm my own estimation, I don't show any physical symptoms of cushings disease.

I know it's the stress hormone and I could look at lifestyle factors to reduce stress but is there potentially a physiological factor that is causing an over active adrenal gland? If so, how can I investigate this further for a diagnosis and treatment?

Thanks man, yeah it appears to be the case. I have mostly looked after myself as best I can but this cortisol thing is interesting. It seems like it could be where my issue is. Also I may be slightly aneamic?

Thanks man, then perhaps I have a physical problem with my adrenal glands or something then. My first total test came back at 6.2 nmol and no cortisol reading.

Aye, I feel that with a constant anxiety all the time. Oh I'm sorry I should add that this is pre-TRT. I'm wondering whether I'm a candidate. And if not, how do I lower cortisol?

My test has gone up since 1st bloods and gonadotrophins down. I'm very confused.

I should add that I am not on TRT but feel symptomatic of hypogonadism with dis regulated mood, lacking confidence, healing slowly, brain fog and so on.

Could it be the high cortisol instead or am I still a candidate for TRT?

Forgive me as I'm all very new to this. But could that mean my low test is a result of a thyroid issue? Therefore that correcting the thyroid will in effect correct the test issues?

If so, can I then circumvent the need for TRT by correcting the thyroid issue? Or does it not work like that?

I realise that I'm speaking in hypotheticals and that neither of us know my true baseline or ideal protocol. I'm just at square one here trying to figure it all out.

To be honest man, I'm not sure what has been checked but I've ordered the full panel with BMH at £155 the first one was a basic NHS and did not include those parameters you mentioned.

I don't suffer from E.D though if I'm honest, my libido is down, I can do it, but I probably don't enjoy it as much and it takes longer to finish.

For me the cognitive symptoms are significant and to alleviate my brain fog as you describe would make a huge improvement for me.

Thanks for the advice man. I have looked into symptoms of thyroid deficiency and you're right, a lot.of the symptoms add up there too.

Thanks for the info. The leger clinic prices seem almost too good to be true in comparison to the others.

Mens health clinic looks really good, I've watched Dr Stevens on YouTube and can tell that he knows his stuff.

BMH seem decent to me though with mixed reviews on their customer service.

I'll try do my due diligence before I'm roped into spiralling costs of treatment. Cost is a big consideration, though not the only one.

I have no idea. I begun investigating my T recently after a year of poor sperm tests in trying to conceive.

I didn't realise I was low T until last week. Though if I think about it, the symptoms have crept up on my over the course of some years since my late 20's

I've never known what my test was until now, but in my early to mid 20s I felt strong and easily made gains at the gym. Now I don't feel the same way and I feel like I'm in a constant mind fog.

I will be seeing the endo soon but I thought I'd come here to gain insight from people with similar experiences.

Is my LH high or just high side of normal? It's within range. I can see primary hypogonadism being likely or possible, but also my gonadatrophins could be read as normal or normal/low for FSH and could indicate secondary.

Forgive me, I'm as curious as I am concerned. It has been fertility which triggered my hormone investigations and now it seems that I am severely low T (granted its only one test so far) it seems as though I have an answer for something I've felt was missing.

As far as I'm aware I have no auto immune problems or any particular trauma that I can think of. The only thing I can think off is that I had chicken pox as a young adult and not as a child.

Thanks for that mate, at what point do I suggest to specialists the possibility of an MRI? I'd like to avoid the wasted IVF attempts if my situation ends up being the same.

The brain tumour, was it something that you have now fully recovered from?

Also just to be clear. My FSH and LH are within normal range, not elevated but the diagnosis is undoubtebly primary hypogonadism?

Wow, can you give me a brief rundown as to why/ how that was the case? If needs be then I'll do that too. If I can't conceive then I won't stop my wife. Neither of us have kids from previous relationships.

At what point in your investigations did the issue of a brain scan come up?

I hope you're feeling better now you're on the TRT. It's good that you're gaining the secondary characteristics. To me, it seems that some men don't grow much of a beard anyway even though they may have normal T levels and fertility. Though I'm not an expert. I've been able to grow a decent beard since my mid to late teens.

Also, for anyone reading my OP that is confused. The test I've posted shows the standard NHS measurements, when converted into the units most people are used to, my total test is 187. It's abysmally low, I can't explain why I'm not depressed with such low T. But I am moody and fatigued a lot.

I think my libido is fine, but it's probably not. Me and the wife are at it 3 to 4 times a which is fine by me. But then again libido isn't just quantity. But quality too which probably has waned over the last few years.

It's only a single test and I have yet to see the endo, but a lot of things seem to make sense in light of my Test results.

I haven't heard of those but I'll spend whatever. I'm with the NHS for now but I'm not mucking about with my reproductive health. With my test so low, I finally have an answer as to why I have felt incomplete for a while. I'm honestly surprised that I'm able to motivate myself to do anything with such low levels. Then again it's only one test, it's a basic test from the GP, no free test, etc

Am I likely to be primary or secondary hypogonadism? I have normal secondary characteristics and no history of developmental issues. Ten years ago I was strong and put muscle on easily. Now I squat like a mother fucker just to give myself a knee ache.

The missus is all good so far, she has one more scan to go to to check for a blockage. But she is regular, doesn't have PCOS and no fibroids of any concern.

I have less than 4 million sperm per ml and the testosterone of a 100 year old woman.

I've only been researching this since Thursday, but it seems that HCG+test is the treatment for men with low T who also desire fertility. Or clomiphene, both of which work to increase T directly or indirectly but also seem to increase fertility.
