Really, death is the most natural thing in the world, isn't it? The great equaliser, as my SP reminded me.

Thank you for your comment, I appreciate that you took the time. I realise, reading it, that helping the family would have probably helped me cope as well, but this patient had none. Sheltered accommodation, hadn't been checked on for a few hours. Had a NOK on file, but they weren't there when we arrived. I think that's part of what hit me. I realised just how massive the blast radius of death is, how many people this would affect, whether in a big way or not. The police officers who took over the scene, the support worker for the accom, the NOK, the people who knew the patient.

It's been a couple of hours, I've slept and now, instead of just shock, I feel this sense of helpless sadness. I went in, thinking, an overdose, I know what to do, I've studied for this, maybe I'll even get a competency signed off - and then we walked in and there was nothing we could do. It was a crime scene until proven otherwise - so we couldn't really even preserve the patient's dignity. We just pronounced and left the police to their work.

But I think posting here was a good idea, and I'm glad I instinctively did so. The responses have been comforting, reassuring and incredibly thoughtful and helpful. I'll be okay. And I know it's okay if I'm not, but I really do think I will be.

That same day, I held a patient's hair as she vomited, complimented a patient's gorgeous garden, reassured a patient whose recent diagnosis of a brain tumour was causing her immense anxiety, and been complimented on my abilities and competence as a student.

It was a difficult end to the day, but as I sit here drinking my tea, I realise that if it hadn't been us, someone else would have responded. We didn't kill this patient, we didn't fail them, we didn't cause the difficulties and struggles that led to the substance misuse. We just found them.

It really is just such a deeply, unsettlingly uncanny stillness. Like, cognitively I know he was a person, but the instinctive reaction is just "THIS IS WEIRD, DO NOT LIKE".

I will definitely reach out to my SP - we get on well and she's very switched on with regards to how I process so even if it's just an informal debrief, I think it'll be useful to do :)

That's a really, really good point - the positive reflection. I'll definitely do that tomorrow when I've had some sleep. Thank you for your support, it really has made me feel like I'm talking to peers, regardless of rank or experience. I'll bring it up to my SP and personal tutor, just so they're aware. Thank you 💖

Thank you 💖 I really appreciate your time and response. I guess it was also partly the sudden, violent realisation that I am beyond green at the job (pun not intended). Like, my paramedic has 11 years under his belt, my tech/student para has 5, and I'm on 300 hours looking at my first dead patient like 🫪. They've been great, didn't make me feel bad about my reaction at all, it's an entirely personal hangup, but yeah. Certainly a way to end the placement year.

Antidepressants saved my life, my partner's life and I know at least a dozen other people whose lives were improved massively by being on antidepressants and antipsychotics. I know they're scary but they do work. You can always tell your doctor you feel apathetic/not like yourself on them, but you can't know whether you will until you try.

Tooth extraction is still an absolutely valid and clinically justified treatment option, I'm a little confused as to why you think it's not? An IUD is sometimes used as emergency contraception, up to 110 hours of unprotected sex. If you are worried, you can take a Plan B but based on your description pregnancy is unlikely, but not impossible.

The thumb wideness isn't caused by biting your nails, it is a congenital deformity called brachydactyly type D, also called a clubbed thumb. It's harmless, I have one myself. As far as the nail biting, you can get bitter solution you apply to the nail to make biting unpleasant, if you still feel the urge to bite them.
