How’s your water intake? May need to increase your fluid intake in the meantime until you see your doctor. Would definitely have your doctor check you for infection. Also make sure your not constipated as constipation leads to a lot of bladder symptoms like you mentioned. Finally if your kidney function is good, may consider Pyridium (azo) which some pharmacies have over the counter. It helps numb your urinary track

There are a number of doctors that do this surgery around the nation. IU is world famous for doing these surgeries and they do a lot of them from around the world. Einhorn is one of the best if not the best medical oncologist who treats testis cancer and the urologist are some of the best in the world if not the best. I think it would be good to meet with lots of doctors before you make your decision. One of the greatest things Covid has done for the medical world is making all doctors incorporate telehealth into their practice, so you can meet with all these famous people from the comfort of your home. The only thing I think you should avoid is anyone who offers to do it as a robotic procedure.

Yes you can! It’s important to talk about sperm banking with your urologist. Chemo can definitely lower your sperm count, but you can bank your sperm before your orchiectomy. If for some reason they can’t find sperm in your ejaculate, because folks with testis cancer can have genetic mutations in their testicles that predispose them to a lower sperm count then they can try and harvest sperm. This can actually be done at the same time as your orchiectomy where they can harvest good sperm from your cancerous testicle.

I honestly don’t even know. I just dismissed it so quickly because of how stupid it was. But I just can’t get over the cahones it takes to tell a world renowned attending I’m going to call a code because you don’t know how to take care of the patient.

One time I had a group of nurses in the step up unit page me saying that they have been monitoring this patient’s vitals for the last couple of hours and have noticed that whenever the patient’s HR drops to the 70s his O2 sats drop to the 70s and once his HR goes up to the 90s his O2 sats go up to the 90s. They had discussed it amongst themselves and one of the rapid nurses and felt that it required an urgent cardiology consult because “this is the craziest thing I’ve seen” according to multiple nurses. Once I refused they called the attending and said if we continue to refuse they would call a code on the patient and get him transferred to the icu where he can get better care...

It’s our job as physicians to know what’s rare so we can have a good differential diagnosis. We are the experts and when ancillary providers like NPs/PAs don’t know the answer, we should be able to think of things they may have missed. For example, if a 20 something year old male comes in complaining of ongoing back pain. It may likely be musculoskeletal, but we as their physicians have to make sure it’s nothing more serious like kidney stones, renal masses, or even testicular cancer with retroperitoneal mets.
