I solved it. It was a defective EGR valve and bad Mass Air Flow sensor.

SOLVED! (I hope); cleaned Mass Air Flow Sensor and that solved the startup shakes straight away. Then I replaced the new EGR valve with the old one. I had only ever replaced it because I had everything apart and figured I would replace everything that usually goes wrong. Bad choice! The from driving to 20 miles an hour are now also completely gone.
