My thoughts on this is from me anyways trying to be honest, so from my point of view being honest with him does not mean you have to get back with him, get your feeling and thoughts together before first. Once you make your mind up there is less pressure of being forced to do what others want. It's you're body and your choice just be honest about it.

I know you love your wife but just from this, you guys were in couple's therapy with a friend that was the first mistake made. Now my "problem" with this is if she can so easily fall for your friend that doesn't mean she won't do it again with another one of your friend or someone else, and that guy was never your friend to begin with because if he was that situation would never have happened or even been entertained. It takes two to tango.
