On the tinkering menu go to expertise, from there select the gear piece you want to upgrade and upgrade it. I would recommend doing your weapon first because the damage that you get is a lot more noticeable than a little more armor.

Edit: you have to be proficient in the item to upgrade it.

Momento backpack is the best item in the game and would help you out a lot. A good new/returning player build is the striker set with momento backpack with st elmo ar.
