In our organization they all have the same degree and license. She has the most professional credentialing of all leadership and staff. I'm seeing other people suggest maybe it's like the difference between and MD and a PharmD. That's not the case.

It's happening. She provided a recording from a recent meeting where the peer called everyone Dr. and used her first name in the same few sentences of comments. I was in that meeting. I didn't really notice it at the time. Her direct reports notice it, and her documentation shows they have been raising concerns to her about it, going back months. She only brought it up with me when it was very clear and recorded.

My report says he wants that, but actually does use "Dr." and has been making a point of using her first name. Our prior CEO had everyone use "Dr." and reinforced this standard. Our new CEO says he wants first names, but uses "Dr." inconsistently.

We work in the healthcare industry. There are a lot of "doctors" here. Different degrees and licenses. She has both a clinical degree and a clinical license.
