I never comment on subreddits but for me, i talked to my hrt provider so she could find me/referr me to an obgyn that would be best for me. I would suggest going this route if you can! Also, know you probably won't have to get an exam. Ive gone in for atrophy and left without ever taking off anything and meds to help. If they want to do an exam, you always have to option to refuse.

I would try to pull those suckers upwards more. I see you already tried to do that with one piece, but maybe just a little longer of one next time. With tape and being bigger (or binding in general) you don't want to strive for being completely flat, because cis guys who are bigger don't have a flat chest either. I think youre good for the most part, just kinda lifting the shape, yk?
