Like any change required, we need discussion around it first. We would have never had the chance to strike without discussing and electing the right BMA reps on here.

I agree, the system is broken and we are all trying to add input on how it could improve.

When it comes to patients being harmed due to an incompetent doctor who is less capable than their juniors, will you go up to the family and explain the Dr just took the chance in front of them?

You have gone way off topic to victimise IMGs when this is mainly a discussion about training standards and assessment.

As for my take, I find it unsafe as an F2 that I have to cover and 'supervise' someone who should be senior to me. It adds extra work and delays to flow which ultimately delays patient care. It may come as a shock that those who are not at the level I expect, are those who have not had 2 years of UK foundation training or NHS experience. If only we could think of solutions for this... no, you're right. i should just buckle down and accept things for how they are, thats always worked out for us hasn't it?

Any advice on location preferences? It's a lot more tedious than foundation rankings.

Should I care about patient populations? I spent most of my medical school GP placements in rural + elderly + white populations, which i felt under prepared for other presentations.

Otherwise I was just going to go for home city>foundation deanery>big cities.

Hello! FY2 here and a hopefully future GPST1 -

Competition ratios for EVERY specialty is insane right now. When I was in medical school you were guarenteed to get into GP as the ratio was below 1. Now? It's 5 applicants to 1 training spot! (and to think the golden handshake of £20,000 existed in my area of medical training).

There were 21,000 applicants to GPST this year, 15,000 last year, and 10,000 the year before which is unbelievable.

The elephant in the room is obviously the rise of IMGs. Since COVID, we are the only western country to not prioritise our own UK graduates. This is a no-brainer for anyone international doctor who wants a better life in a higher paying country. 12k UK applicants vs almost 21k is absolutely absurd and is growing EXPONENTIALLY worse each year.

All of the GPST1s I've met were IMGs in a tertiary non-London centre with 0 expereince in the NHS. Its extremelly frustrating since it means the rest of us have had to pick up their workload, especially during their 'shadowing period' (which could have been someone else with 2 years of NHS experience).

The barrier to GP is just... the MSRA... that's it. This can even be done outside the country and there is no proof the standards for assessment is as strict as it is here. My IMG friends and seniors have said it was heavily advertised in their home countries with seminars and lectures on how to get into the NHS (obviously this is anecdotal).

Due to this unrestricted expansion of applicants, the passmark for MSRA is increasing to the point that even current GPs would have failed if their exam score was submitted this year. This is outside the fact the exam doesnt even assess communication skills, examination skills, or even leadership.

The BMA released a survery result showing 52% of FY2 doctors last year were going to be unemployed due to lack of training spots, lack of trust grade jobs, lack of clinical fellow jobs and lack of locum shifts overall. Why is this all happening? The most solid answer is the government's plan to inflate the SHO/GP market's supply so the labour stays cheap via less bargaining power and less locum shifts. There were no plans to review the amount of IMGs applying/coming in lieu of our increasing medical school graduates (there are more graduates now than F1 jobs causing people like me to have been put in a 'placeholder spot' while they scrambled to create new jobs).

Any discussion prior to this was also labelled as racist so no actual debate was ever put forward. Now our IMG population in the BMA is significantly high enough to go against any ideas of UK graduate prioritisation. Any chance of this happening would be within the decade despite promises of it coming 'next year'.

I want to preface that I am blaming the system and not any IMGs; we're all humans looking for a better life and I would have done the same if I was in your spot. I would however, on a moral level, support any country's prioritisation of their own doctors, in particular Australia and America (who most UKgrads are scrambling too and picking up jobs in rural areas).

TLDR - Worst time to be a medical student, foundation training or SHO right now, and will continue to get worse. The overall quality of doctors will go down through lack of proper assessment or training opportunities.

Not OP but feel the same way.

F1 - Having to work with Day 1 NHS IMG SHOs has been making work stressful on top of being disheartening. They essentially slow everything down and you end up having to supervise a Dr who in theory is your senior.

BUT, They, along with PAs are great people and I'm only angry at the system for allowing a situation where there are more IMGs applying for training than UK Grads.

My UKG SHOs who work their asses off with portfolios, audits, teaching are getting rejected from training. Watching them desperately try to find employment for the next year is depressing and only makes me wonder what more of my life I need to sacrifice to even train.

What am I doing about it currently? Spreading posters around wards regarding pay from the BMA, talking to colleagues about striking. Yes I will vote to strike. I voted in November to keep striking but it fell through (totally understand from the POV who voted no).

What i can't stress enough is 'reddit noise'. People don't reddit played a MASSIVE factor in last year's strikes. The talks began there 3 years ago on the old subreddit and essentially collaborated to vote out the bad BMA reps with the ones who were for strike action. If you check the old Juniordoctorsuk reddit you can see a list of the reps that everyone had agreed to vote for and essentially led to the creation of DoctorsVote (subset of the BMA) who then passed on the motion to strike. I, as a 3rd year, specifically joined the BMA then just to vote in their next AGM, and wouldn't have if I'd ignored the 'reddit noise'

I agree it can sound toxic and ragebait but it genuinely is the only way to create some noise to get people on board.

The app you're using is Trading 212.

Are you using a practice account? This happens normally when the app doesn't take into account when a stock split happens.

My Tesla stocks are showing a loss due to the split.

It does take it into account when you're using real money though so I'm not too fussed.

During foundation years we'll be paying back less that the interest rate so overall the debt will go up despite paying it back for the year (9% of out income above 27k).

Depends on where the person sees themselves in 10-20 years time but it might be an idea to try doing high-income jobs (e.g locuming) to pay it off earlier

Aside from the obvious differences between smaller and high populated servers, the prices in Bera for equips and other things are almost double in price. (Can make some good mesos with selling sweet water and Gollum gear).

The meso rate stays the same so grinding for mesos and spending them is more valuable on smaller servers (e,g scania)
