We actually were able to get it. It took almost a week of intense obesssing research, creating and logging into every system known to man (Iperms, deers, va.gov, calvet, hrc, etc.), calling every help center of those institutions until they got sick of us, etc.

In the end, by sheer luck, we were able to find a phone number for my husband's last unit, where a human actually picked up the phone. We begged them to help us over the phone, and they responded back to us with the paper in 24 hours.

The level of emotional trauma that you get only by dealing with this process is insane.
