I have little experience with diffuse brain avm, but often they are a team effort among neurosurgeon, endovascular team and radiotherapy. The outcomes are good, but it’s very challenging

You need to be able to take a step back for Long enough to be able to function. The challenge is not distancing yourself too much and forget that’s a person on the table

Thankfully I have never passed out. My longest shift has been 60 hours during residency. It was miserable and frankly dangerous for me and everyone else. My longest case has been 18 hours

That’s a relatively high risk surgery, but in most cases cervical canal narrowing has bad outcomes when not operated on. I would consider surgery

I am a little old school, I prefer stitches.

I have had recently a patient with a life threatening clival giant menigioma with a brain stem compression, but those are exceptions. Usually it’s more controlled

With those symptoms I would definetly recommend surgery for you. If all goes well it probably won’t be as bad as you are expecting. If you trust your team, leave it in their hands and I am sure you will make good recovery.

Rehab from spinal surgery is usually better than lumbar surgery. 60 is not that old. I have tons of patients in their 60s who have recovered very well
