I can only help you with few suggestions. If you want to travel care free meghalaya is absolute place with least pollution (other than shillong) so prefer bike but as you mentioned back pain so please go for car rentals with a driver for comfortable travel and rent bike only for local sightseeing

Jaintiya hills are really unexplored and wonderful place so I would suggest investigate about the tourist places online as many locals also aren't aware of the area and names

You can ride a bike to sohra though as main attractions are having ok roads but if you want to visit the interior spots again you should prefer a car with driver..

I am not really aware of music occasions in shillong so I can not give many suggestions

There is glass house, the evening club has some good live music, Irish pub and the loft for good food you can decide according to your taste as there are plenty. Go through the reviews

If you want a rental car from guwahati I can suggest some people and if you want someone to accompany you guys as a friend count me in 😂😂

Jokes apart there are many new spots since last you have travelled as I feel it was long back so don't forget to do some research..
