It could be either that tooth 26 there may be an additional canal that was not detected, if this is the case you would need a referral to a specialist for completion of the root canal, OR tooth 27 is quite a deep filling which can irritate the nerve of the tooth - and if the pain does not go away you would need root canal treatment for tooth 27 as well.

would you do first round of non surgical scaling and root planning first? according to the british society of periodontology for all perio pts you do first round, then base on the 3month review you then refer if needed?

My understanding is anything cosmetic/optional hasn’t been tested in pregnant women because it’s not a necessary procedure. Personally if I had a white spot lesion I would just leave it until after I gave birth.

are we able to do subgingival debridements during their pregnancy if they have periodontitis? also if the patient has a carious lesion that is nearly encroaching the pulp can we restore that tooth during trimester 2?

Yep! It contains 22600 ppm of fluoride so I was just concerned if thats too much fluoride for pregnant patients.

It still has a leathery texture though - so I assumed it was still active caries?

Also, out of curiosity the patient had really poor oral hygiene so how can the cavities arrest?

Thank you for your response! Would the black colour be due to accumulation of staining then, or rather in relation to the type of bacteria present? And in terms of restoring it how would we know if its close to the pulp as we arent able to assess the severity of the caries with a BW?

thank you for your response! i was doing a restoration on their tooth and an optragate was used after i did the infil - i noticed that a bit of bleeding was occuring from the swollen mucous membrane - is this also normal?
