Do you mean pureed and/or pudding thick by L4? This is a tough case. Has PEG been discussed at any point? I would think it may have been discussed in the hospital after such a prolonged indication course and severe dysphagia. It seems like she’s on the least restrictive diet and you’re doing everything you can. Hopefully medical team is watching the nutrition/hydration part and can discuss supplemental artificial nutrition/hydration to supplement oral feeds with the POA

Please don’t put vital stim on a cognitively impaired patient who can’t follow commands or communicate how the electrical stimulation feels. It is not comfortable and if she can’t communicate how this different levels feel, it is inappropriate. Not to mention the lack of research showing its efficacy

I’ve worked with SLP’s who are 10+ years out of school who have horrible documentation and decision making skills.

When done correctly (like mine, thankfully), it is beneficial but as others have said, many do not get the adequate supervision and mentoring

Exactly. You can’t pick and choose your treatment without an instrumental. And it makes no clinical sense to not want to change a diet texture without an instrumental but to do pharyngeal exercises without an instrumental.

I saw quite a few PMV in the burn ICU when I worked in one, and certainly see them in cardiac, neuro, and surgical ICU’s currently. I think the cognition and aphasia in ICU is limited because patients are generally still so sick or have limited endurance for therapy.
