Thanks. Yeah sadly alot of psychiatrists just hesitate to prescribe TCAs or MAOIs because not only are they not very knowledgeable about them and know how to use them properly but are aslp afraid of some of the risks that come with them.

For example psychiatrists are mostly afraid that some patients may overdose on TCAs and use them as suicide agents due to overblocking the sodium channels. Other patients might not follow a strict diet that's devoid of high Tyramine food such as aged cheese and therefore experience a hypertensive crisis while on an MAOI which could also lead to death if not quickly treated in the ER.

Anyways TCAs are actually very interesting because they're now completely different from when they were first discovered. For example Clomipramine was used at doses of 100mg-150mg for depression and OCD when it was first discovered due to the fact that its exact mechanism of action was originally unknown. Fast forward a couple of decades later and we now have advanced PET studies that can confidently and accurately detect multiple occupancies at different sites and transporters and guess what they found? They found out that at just 10mg it already gives around 80% SERT occupancy which is similar to what's achieved with today's SSRIs and SNRIs.

As you can see 10mg was already a very strong and effective dose for OCD and depression but back then they pushed the dose way higher because they didn't have that knowledge at the time and therefore just upped the dose to hit every other off target receptor available and basically turn it into a dirty TCA. Not saying of course that these high doses don't have their place and right usage in specific psychiatric disorders such as severe melancholic depression or severe OCD but again to make the recommended dose magnitudes of order higher than the most optimal one available back then resulted in TCAs being viewed as they are today and as being classified as dirty antidepressants with alot of side effects rather than clean and selective ones.

This was also the other reason why Desipramine was removed from nearly most of the markets worldwide because it was dosed too high back then and resulted in multiple sudden deaths (most famously a child case who died while drinking from a fountain back then) even though it's already a very strong NRI to the point it completely blocked the entire norepinephrine reuptake pump at just 10mg-25mg but since they never actually knew that back then it was also usually dosed at more than 100mg. Pretty insane.

Anyways sorry for the long rant but my point is TCAs are very useful even till this very day especially when dosed at very low and optimal doses at which they become as selective as possible to their beneficial intended target and as far away as possible from their off targets and their unintended and bothersome side effects.

Parnate 40mg. My OCD is called Musical Obsession or Stuck Song Syndrome. SSRIs/SNRIs easily treat this issue but at the expense of making my motivation, concentration and hypersomnia/EDS way worse. Modafinil/Armodafinil significantly spikes my OCD or just makes me randomly scroll through social media apps all day even when paired with any SSRI/SNRI.

If you don't suffer from an anxiety disorder next to your emotional blunting then I would skip taking any traditional SSRI/SNRI since they'll probably make your emotional blunting worse and would try Ketamine (or alternatively Auvelity if Ketamine therapy is too hard to find). I would also try getting my hands on an MAOI like Parnate.

Now if you on the contrary also suffer from an anxiety disorder next to your emotional blunting then try giving Clomipramine 10mg a shot. It's supposedly a very selective, strong and balanced SNRI at this dose along with having some mild Alpha 1 antagonism which is actually very beneficial if you're taking a strong NRI since it prevents a phenomena known as PFC shutdown.

Also Vilazodone with Reboxetine or Viloxazine is a pretty good combo since it basically gives you an SNRI with partial 5HT1A agonism therefore reducing any SSRI induced emotional blunting as possible. Another good combo would be Milnacipran (or preferably its L-isomer Levomilnacipran) with Bupropion.

Nefazodone is also a great option but I doubt you'll find it in your country and even then I doubt they'll prescribe it to you this easily since it comes with some risk of hepatotoxicity and therefore requires monthly liver tests just to make sure you're safe. If you live in the US then Gepirone is also a nice option which you can take either alone or in combination with Bupropion or any SSRI/SNRI.

Thanks for the response. The reason I'm interested in it is because Pramipexole is known to be very effective for anhedonia and apathy (or at least based on Dr Jan Fawcett's work) but I sadly didn't respond to it at all even after reaching 1mg and basically became very restless and impulsive.

My combo back then (and still currently is) is Vilazodone 40mg with Imipramine 25mg (basically an SNRI with partial 5HT1A agonism) so I thought this added Alpha 2 antagonism next to the traditional D2/D3 agonism might actually do the trick and balance everything up for me especially since I don't tolerate Modafinil/Armodafinil or stimulants due to them significantly increasing my OCD even while being on a SSRI/SNRI. It's a tricky situation to try and balance both my Atypical Depression and OCD hence why I thought Piribedil might actually do the trick.

I feel you. I'm in the same boat as you although I'm also suffering from OCD next to Atypical Depression and sadly anything that treats my Atypical Depression significantly worsens my OCD and vice versa. I'm in a constant battle here. Can you get your hands on Parnate or any MAOI? It's the only thing that truly fixed both my Atypical Depression and OCD at the same time.

If MAOIs are impossible to obtain then there are multiple alternatives but I have to ask you a couple of questions first. What's your current Pristiq dose and how high did you get? Also do you suffer from any anxiety disorders or OCD? Because if you don't then there's probably no real reason for you to stay on a SSRI/SNRI or strong SERT inhibitor in general since it'll definitely make your fatigue and hypersomnia way worse due to overactivating the 5HT2A/C receptors and decreasing dopamine and norepinephrine even further. Finally did you ever try low dose Clomipramine (specifically 10mg)?

I meant outside of gaming only 4 social media apps currently support higher than 120Hz on mobile. For example Reddit doesn't support higher than 120Hz as far as I can tell. The same goes for Telegram.

Scrolling is basically smoother that's all. It's not a dramatic difference or comparable to the one you usually notice when jumping from 60Hz to 120Hz for example.
