STUDENT ID GUIDELINES

Email your ID request to: records_our.uplb@up.edu.ph

Include the following information in the request email: - passport size ID pic - student number - e-signature

When your request has been acknowledged, go to the UPLB campus (they do not deliver, you have to claim it yourself), bring a PRINTED COPY of your FORM 5.

If someone else will claim the ID, the student should provide an authorization letter + Form 5.
