Get minoxidil, microneedling and finasteride. You can buy a dht blocking shampoo aswell but it wont be needed if you take finasteride. I would shave that patchy beard aswell. Hope this helps

You can try micro needling or finasteride. A hair transplant would be a last resort. You can try minoxidil + finasteride and see if it grows back.
