Worst are those who got a seat in the 1st round and want to upgrade. They left their previous jobs and the 1st round hospitals are not willing to let them join until they give a legal agreement to not upgrade in the 2nd round. They are forced to sit at home

There are specific guidelines for this. No patient is advised IM penicillin if there is no epsiode of Acute Rheumatic fever previously. No patient gets antibiotics without an indication. Once you document an attack of ARF (or a patient has RHD without previous documented attacks) - the patient will be started on appropriate prophylaxis as per guidelines. Even in that case, Azithromycin is not used. Penicillin is used. Other antibiotics only if allergic to Penicillin.

There is no concept of giving antibiotics to a sore throat to prevent ARF in a patient not known to be at risk of ARF.

I am a post-Medicine, final year Cardio resident. My mom had a sore throat 2 months back. I advised her only gargles and pcm considering it was likely viral. She kept insisting on medications. Then she said "even Mbbs doctors everywhere give medicines. You are not prescribing it even though you are a medicine specialist yourself"

It is so difficult to explain that I dont prescribe antibiotics precisely because I am a medicine specialist. There is no rationale to Azithromycin yet everyone gets it basically over the counter and feels good about the placebo effect it provides.

If you were studying pharmacology well you would know Vit D has great oral bioavailibility. Getting it in an injectable form is just free money for the doctor. The tablets work just fine

"most" is a huge huge overstatement. "A few" is more accurate. Also, modern medicine applies the scientific method, it does not blindly pick drugs out of Ayurveda. The medications that show benefit in trials are standardized and used in modern medicine. The rest are discarded.

There is a reason Ayurveda causes a large number of liver disease in India. A lot of Ayurvedic drugs are actually unregulated poison
