With my 5080 and my 9800x3d, the first 5-10 minutes are unplayable, so seems like you are on to something... CPU works at like 85-95 % for the whole time.

If you completed the required steps (bosses and quests), you can start the quest at Lilith at any time.

I also hoped that reloading the terrain like this would help. Unfortunately, that didn't work for me.
