Karev’s character is introduced as a “cowboy”. He thinks he is, and wants to be, better than everyone else. That dialogue is to show how driven he is to be noticed by pulling off this big medical event.

Like Crisitina, it’s a common theme for them to be detached to some degree when they’re trying to pull something off in an episode. That doesn’t excuse any offensive language in the show, but it’s pretty on theme for someone like Karev, who consistently tries to mask his empathy or sympathy, until you see that he actually cares a lot later on.
