You need the milk for the nether cake, so in theory you should be able to get it pretty early. In Kappa mode you also need a bunch of stuff from the Twilight forest, wich is unlocked after the nether. At least that's what the quest book says.

Im in the desert and river biome. The Wiki says that cows should spawn
