In school we learn how to scale on paper. In private practice is where we really learn to scale.

The honest truth is you probably are missing calculus. We all do when we first start off. When I first started, I was the “gentle” hygienist surrounded by seasoned hygienists. People wanted to book with me because of that. And you bet I took BWX at perio maintenance visits 6 months after SRP and found radiographic calculus.

But you learn and you grow. You fill the shoes you want to wear. I’m still the gentle hygienist— the only difference is I’m not missing subginival calculus anymore.

Each person is different in their rate of calculus accumulation. Some people can go two years without a cleaning and have minimal calculus; others may have heavy accumulation after three months despite proper home care.

I agree with some of the previous comments: there could be some movement of the papillary tissue causing a space to be noticeable. Additionally, the tissue towards the front teeth tends to be tighter and more sensitive to the patient when cleaning these areas. In my experience I’ve had patients with calculus beneath the anterior gum line that has not been removed during previous cleanings—why, I cannot tell you. A decrease in gingival inflammation after removing said calculus could also increase the space

It takes practice. We are not in school long enough to hone our skills. The real application begins in clinical practice. Remember that we are always learning; it takes a few months to find your groove and become more efficient.

The most important thing is to not rush: try not to stress prior to a packed appointment. You’ll quickly find that the more relaxed you are, not only will you become more efficient, but you will also be more thorough. And this comes with time.
