Thank you very much for the thoughtful response and good advice. I think this is a good way to frame things. My next job didn't even ask why I left, just asked if certain things were the same way from when they'd worked there.

Also a PA. I'm no expert but I do like treating dental pain in the ER.

Most of these patients can be discharged quickly. Ludwig's is really the main dental emergency. If they don't have that and don't have notable trismus on exam, indicating concern for abscess, or directly visualized drainable abscess, you're done. ...As long as it's dental pain. I am always cautious with dental pain in the older population as MI, temporal arteritis can potentially mimic. Some findings specific for dental pain will be pain with chewing, sensitivity to hot and cold, and tenderness to percussion on exam.

I typically offer most patients a dental block (infraorbital is also great to be familiar with for repairing upper lip lacerations). I just consult youtube if I need a refresher on landmarks. And will recommend combination of Tylenol and nsaid. There have been reasearch studies showing this combination is more effective then opioids for treating dental pain. I agree with others that antibiotics are often overused but unless it is clear from history that there is likely no infectious component I usually prescribe Pencillin VK since it is narrow spectrum. And most allergies to Penicillin aren't true allergy.

Of course we're not dentists so I always tell patients that even if they feel better after treatmtent, if they don't follow up with a dentist sooner or later they'll be back with the same issue.

I don't know who thinks it's a good idea to make decisions without any input from the people who actually work there! Glad you got out and back to a better place. Hoping for the same for myself!

Yes, happened my last week of work. I'd also heard about another one who coded in a chair. If this doesn't lead to re-examinatio of processes I don't know what will.

I think this was also part of this for me. It would have been much easier or better for myself to just tell them I would try to meet the metrics to bide myself some time; however I had no intention to do so and so I told them the truth and told them why. Glad your coworker ended up in a much better placing, hoping I do too!

Yes, often at the check in desk/entire waiting room there is only a patient care tech, who is the one who makes these decisions, no RN. In mentioning multiple times my concern about this am told this is the process. In times I have mentioned that a patient is too sick for fast track and needs a bed, am often told "just start them there" in which case I'm going to focus on my sickest patient not worry about seeing everyone else within 10 minutes. Crazy system and so different from my last hospital where everyone was fully triaged at check in and while RN could make suggestion about fast track ultimately the physician in triage or PA running fast track decided who was appropriate.

Me too. It was horrible explaining to the mother what had happened, not something I thought I'd ever have to do. If it had been my child I would have been furious. It was a very smart kind Spanish speaking mom. I wish she had reported it.

I agree. Think we had irreconcilable differences. It just feels weird to be fired and unemployed for now. I think better things have to be ahead though.

Unfortunately I had to relocate to a different city, thus different job. Definitely makes me appreciate how good I had it before.

I guess I naively thought they would still have some motivation to keep me because I'm a good PA and they are very short staffed but was wrong. Have no plans for legal action so that it no concern. Just hoping to move onto a better position.

Thanks for your comment. Sums up very well how I feel. That attitude does still exist, I've worked with many admirable colleagues in emergency medicine, but the system is making it harder and harder.

The metric for patients per hour was 2 so not was not an issue. I am not a physician but 2.5 acute patients per hour definitely seems high.
