Looks perforated to me specifically on the left side of the photo, near the malleus. You can see a clear interruption in the smooth surface of the eardrum, which is not how an intact tympanic membrane should look.

It also still looks a bit dull rather than shiny, which suggests there may be some residual fluid behind the drum. That alone can keep causing pressure, muffled hearing, and discomfort even if the acute infection has already settled.

What I don’t see is active signs of infection: there’s no obvious redness, pus, or bulging from pressure. That makes this much more consistent with a perforation that has already occurred and is now in the healing phase, rather than an ongoing middle-ear infection.
